Thank you for choosing North Bend Medical Center and for your interest in becoming a new patient with us. We’re excited to partner with you on your healthcare journey.

Getting Started

To apply for a primary care provider, you’ll need to complete a New Patient Intake Form. This can be done electronically by clicking the link below, or you can print and mail or hand-deliver your forms to:

Coos Bay, OR 97420

If you are new to NBMC, you’ll also need to complete a Medical Records Request Form so we can obtain your records from your previous providers.

If you have multiple providers, please complete a separate release form for each request.

A copy of your photo ID will need to be uploaded with the form to facilitate the request.

What Happens Next

Once your forms are submitted, they will go directly to our Patient Access Team.
You can expect a friendly call from our team to review your options, discuss possible scheduling, and provider availability.
