The Anticoagulation Clinic provides education and monitoring services to people who have been prescribed a blood-thinning medication (e.g., Coumadin) after a heart procedure or irregularity. Our clinic is staffed by registered nurses with special training in managing anticoagulant therapy.

During visits to the Anticoagulation Clinic, a nurse takes a small sample of blood to determine how long it takes the patient’s blood to clot. Providers have provided guidelines which allow our nurse to make adjustments to medication doses based on the results of the test.

Our Behavioral Health department follows the primary care integrated behavioral health model. This is a progressive model of care in which behavioral health clinicians are embedded into our primary care practices to assist the medical providers with using brief, evidence-based interventions from a trauma-informed perspective.

We work in collaboration with your medical team to assist with numerous challenges and wellness goals including: smoking cessation, diabetes management, weight loss, anxiety, depression, PTSD, grief, stress, coping with illness, and much more. Our highly skilled clinicians are able to see both pediatric and adult patients.

Our behavioral health clinicians are focused on helping a large number of people achieve very specific goals, which is a departure from the more familiar long-term psychotherapy or counseling. This means that our visits are less frequent, shorter in length, and target specific goals for treatment. Our services are designed as an additional option to traditional therapy and are covered by nearly all major insurance payers, including the Oregon Health Plan.

There are many factors that impact your health, and our Care Management team can help sort them out.  As a vital part of the team, Care Managers think about what patients need when they aren’t in the clinic.  Care Managers engage with patients and their families to develop a customized plan of care built around the patient’s goals, preferences, and priorities.  They help identify barriers to meeting the plan goals and then provide education or referrals to other resources that may be needed to ensure success.

Our Care Managers provide a host of self-management support education, both individually and in group settings.  We offer engaging courses in diabetes education that includes family-based learning around healthy food and lifestyle choices.  They also offer sessions focused on living a healthy life, end of life planning, and host a weekly walk-in blood pressure clinic.
